Final Forms is open for registration of new students for the 24/25 school year.  

Please don't wait until the last minute to enroll. There is a 3-5 business day records processing time between the completion of enrollment and the first day a student can attend classes.

We cannot finalize enrollment without all of the necessary documents and Final Forms completion.

The Welcome Center follows the district calendar and all weather-related district closings and delays.

The Welcome Center is accepting new enrollments electronically through Final Forms Please see the instructions below, including a list of documents needed to complete your enrollment. Parents are not required to present documents in person to our Welcome Center office, but we reserve the right to request in-person presentation of original documents at any time.

We’re open during the summer!  Please don’t wait until August to register for the upcoming school year, as the Welcome Center experiences a high volume of traffic and delays can occur. 

**Enrollment is only open to residents of the Princeton City School District.  All claims of residency are subject to investigation, and fraudulent claims will result in parent/guardian liability for payment of tuition for each child ($11,295.37)** 

In order to register your child in the Princeton City School District, you will need to:

  1. Complete and Sign every form within Final Forms.

  2. Upload all the required documents in the document checklist below.

  3. If you encounter any issues or need to schedule an appointment, please call 513-864-1111 or 513-864-1113.

  4. Your child will not be able to begin school until all information in Final Forms is complete and the required documents have been submitted and reviewed by the district registrar. You will be notified by phone or email with a start date once approved for enrollment. 

You may upload any of the documents below digitally in Final Forms

  • Option 1: Upload a File from your Computer.

    • Click the UPLOAD button, locate the file, and upload.

    • Be sure the file is a DOC, PDF, JPG, or PNG.

  • Option 2: Upload a Photo from your Phone or Tablet.

    • Click the UPLOAD button below, take/locate your photo, and upload.

    • Be sure the photo is clear and well-lit.

  • IMPORTANT: Name your file to help administrators identify the document.

Required Documents (All forms must be submitted in their entirety.) 

  • Child’s birth certificate or passport 

  • Parent/guardian proof of identity (driver’s license/state ID or passport/Green Card)

  • Immunization records  (Please see immunization requirements)

  • Proof of address/residency – Three (3) Items from list below:

You will need ONE of the following (partial documents will not be accepted):  

  • Complete signed lease/rental agreement*               

  • Mortgage statement*

  • Settlement statement

  • Property tax bill

  • Deed

  • Mobile home title*

*If you are not on the mortgage, lease or mobile home title, you will need to complete a notarized residency affidavit at the Welcome Center*

You will need TWO of the following in parent/guardian name: (envelopes not accepted) 

  • Payroll check stub (dated within past 60 days)

  • Utility bill (dated within past 60 days)

  • Vehicle registration

  • Hamilton County Job and Family Service letter (dated within past 60 days)

  • Recent income tax return (dated within past year)

  • Voter registration document

  • Insurance policy (dated within past year)

  • Medical bill (dated within past 60 days)

  • Certificate of registration from the Board of Elections

  • US Postal change of address confirmation

*All suspicions of non-residency will be thoroughly investigated and additional documentation of residency will be required*

High School Student Enrollment Only

Getting into the right classes for graduation is important! Providing transcripts or other records with the enrollment application will allow counselors to create an appropriate class schedule faster.  These records can be uploaded into Final Forms with the other required documents. 

  • Applicants with current records will have to wait 3 days to begin classes after enrollment is finalized. Current records include transcripts, report cards or class schedules that show which high school courses the student has taken or classes in which they are currently enrolled.

  • Applicants without current records will have to wait 5 days to begin classes after enrollment is finalized. This will allow us time to request and receive records so that we can create an appropriate class schedule.  

Special Situations

  • Custody Documents (if applicable)

    • Divorce decree and Shared Parenting Plan (must be court stamped)

    • Court issued custody decree/guardianship paperwork (must be court stamped)

    • Current IEP and ETR/MFE. An official request will be sent to the previous school; however, having the documents upfront facilitates a smoother transition.

The Princeton City School District is in the process of identifying, locating, and evaluating children with disabilities who may be in need of special education and related services.

From birth to age 3, a disability is an established condition known to result in delay, or a documented developmental delay.

For ages 3 to 5, a disability is a documented deficit in one or more of the following developmental areas: communication, vision, hearing, motor skills, social-emotional/behavioral functioning, self-help skills, and/or cognitive skills.

For ages 5 to 21, a disability is the identification of one or more of the following conditions: autism, deaf-blindness, hearing impairment including deafness, mental retardation, multiple disabilities, orthopedic impairment, other health impairment, emotional disturbance, specific learning disability, speech or language impairment, traumatic brain injury, and/or visual impairment including blindness.
